The Library can issue to a registered faculty or staff member (the requestor) a proxy card to permit a designate to borrow and access material on their behalf. The card will be honoured throughout TUG. The purpose of the proxy card is to support the requestor in their research by providing full library access to an assistant.
Under this arrangement, the designate does not have access to the requestor's personal file. The proxy will be given a unique file that will be linked to the requestor's file. After the expiration date the Library will then purge the proxy file and transfer the books to the requestor's personal account. The proxy card link will no longer be active. A list of the books transferred will be sent to the requestor.
Your proxy designate will be authorized to perform the following activities on your behalf:
Completion of this form indicates that you confirm you are requesting this card only for the purposes of support for your research, and that you accept responsibility for any materials charged to this account. The card may be revoked if it is used for other purposes.
Please note: the proxy card is only to be used by the person whose name appears on the card.
