Name Release Form

As part of the library’s mandate to facilitate access to information for the greatest number of users, the following policy has been found to be helpful.


Name Release: (in French, Chinese, Spanish)

UW Library practice is to keep confidential your name, address, telephone number, and the title of books that you have borrowed. However, you may waive your right to confidentiality by authorizing the library to give your name, along with your phone number and/or your email address, to any other Library user who indicates a need to consult books which you have borrowed.

You should be aware of the following:

  1. If someone contacts you to consult a book you have borrowed, the method and location of consultation are your choice and, indeed, you do not have to agree to allow the person access to the book at all.
  2. If you do allow someone else access to the book, responsibility for the book remains yours.
  3. You may change your decision about name release at any time by simply letting staff at the circulation desk know that you wish to do so.

Indicating "Yes" to name release means that the Library may release information in your file. If your response is "Yes", please indicate whether we may give out your phone number, your email address, or both. Indicating "No" means that information in your file will remain confidential.
