Assessment Project Form

Use this form to submit a brief review of the assessment projects you have undertaken since January 2000. This will be added to the Library's repository of client needs assessment, which is maintained by the Community Needs Assessment Committee. Contact CNAC if you have any questions.

Once your project is finished, please send CNAC all related documents. For example: project URL/website, proposal, charter, questionnaire, letters, final report etc...
