Good news – our timing and efforts paid off with provisional clearance and essentially minor revisions only (see message below). I can respond to questions 1 and 2 (ii) and coordinate the revised application. However for question 2 (i) I need some more information. Alex – have you confirmed that the website will not be collecting email or IP addresses? If so, Amy and Wish, would you please update the letter to reflect Susanne’s comments. Thanks.

Dear Sharon,
This note is to advise you that the ethics review of your ORE application entitled ‘Library reserves: undergraduate student survey’ (ORE # 11324 ) has been completed, and on the basis of this review, the project has received provisional ethics clearance. Below is a summary of recommendations for revisions as well as any related comments.
1. Comments on the Recruitment Process
(i) Recruiting by email using the information letter is fine. Will the Registrar's Office be sending out the emails on the library's behalf or do they plan to provide you with the email addresses?
2. Comments on the Information Letter and/or Consent Form
(i) If the website will not be collecting email addresses or IP addresses, this information needs to be included in the letter. It could appear in the fourth paragraph. Since participants provide their email to enter in the draw you may want to state that if they choose to enter the draw the email address they provide will be used only for the draw and in no way linked to their survey responses.
(ii) Does the reminder go to everyone on the email list or do you have information on who completed the survey or not? Please provide further information.
3. Comments on the Survey or Interview Protocol
(i) These appear to be fine. Please provide a copy of the final version of the questionnaire to the ORE, once it is available.
4. Comments on the Feedback Letter/Letter of Appreciation
(i) This is fine.
Sharon, this summarizes the feedback from the ethics review of your application. Revised materials should be provided within ten days to the Research Ethics Coordinator in NH 1024 in hard copy or by email to ohrac@admmail.uwaterloo.ca <mailto:ohrac@admmail.uwaterloo.ca> When submitting your revised materials, please highlight the revised section(s), and indicate the ORE number to which the revisions refer, to expedite the ethics review of these documents. Please note as well that the corresponding study may not be started, including recruitment of participants, until the full ethics review certificate has been issued.
