Title of Project:             UW Library Reserves: Submit Feedback on Library Service

The University of Waterloo Library is conducting a survey to assess our Reserves service. Please complete this survey even if you do not use the Reserves service – your input will help in our assessment. There will be a draw for ten prizes of $20 each. You will be given the option to enter the draw when you have completed the survey.

 

Library Reserves are course materials that have been gathered together at the request of faculty members for students to borrow from a circulation desk or to access electronically.  We are interested in how well our current service is meeting the needs of students, and seeking to better understand how students would prefer to use Reserves in the future.  You have been randomly selected to assist in our research.

This brief survey will take approximately 5-10 minutes to complete.  Please go to the following website to complete the survey: http://www.ist.uwaterloo.ca/surveys/library_reserves/index.php

Participation in this survey is voluntary and anonymous.  You do not need to identify yourself by name on the survey. You may decline to answer any questions that you do not wish to answer.  IP addresses will not be associated with survey responses.  If you choose to enter the draw, the email address you provide will be used only for the draw and in no way linked to your survey responses.

 

The data collected from this study will be accessed only by library staff and will be maintained on a password-protected computer database in a restricted access area of the Library.  A summary of the results of the survey will be posted on the library website; notice will be given when the summary is available.
