Subject Line: UW Library Reserves: Faculty Survey
The University of Waterloo Library is conducting a survey to assess our library Reserves service. We are interested in knowing how well our current service is meeting the needs of faculty. We are also seeking to understand better the future of paper and electronic reserves. Please complete this survey even if you do not use the Reserves service – your input will help in our assessment.
This brief survey will take 5-10 minutes to complete.
Although the survey is web based, a paper copy of the survey is available from your departmental secretary, or from Wish Leonard at (519) 888-4567 ext. 5430, aleonard@library.uwaterloo.ca.

Participation in this survey is voluntary. You do not need to identify yourself by name on the survey. Information that could identify you by computer or by email is not being collected.
