We're requesting to mount Stage 1 on the Library's home page as soon as possible. On the day when the new Library's home page is live we plan to implement Stage 2. The survey is to be withdrawn from the Library's home page on December 31, 2005.

We are currently in the process of redesigning our website to reflect the new "UW Common Look and Feel" in order to improve accessibility and usability. Participation in the survey is voluntary and anonymous. Survey data will be securely stored for 2 years. This project received ethics clearance through the Office of Research Ethics. Thank you. On the day the new site goes live we keep the survey on as is, but we change the preamble to:

Proviso: We would like to run the survey on the home page on a regular basis. For example for the month of October or November during the fall term, and February or March of the winter term. The heading and questions stay the same, perhaps the preamble should change slightly.
