Additional Information

The Safety Office serves as an environmental health and safety resource to the university community to assist students, faculty, and staff in meeting their obligations in provision and maintenance of a healthy and safe campus. Information on safety at UW is available from the Safety Office homepage at: http://www.safetyoffice.uwaterloo.ca


Introduction

The University of Waterloo is committed to safe working conditions on campus. “Supervisors and all other persons in authority must provide for health and safety, including training in specific work tasks, in areas and operations under their control” (UW Policy 34). In full support of this the Library has developed a safety manual which was first published in 1985, and has since been reviewed and revised several times.

I encourage all staff to read the manual carefully. A copy of it is readily accessible in every library department and on the Library Staff Web.

Section 1: Elevator Failure

1.1 Reassurance

If someone is stranded in an elevator, it is important for library staff to give reassurance until help arrives. Call out to the person to assure them that you are aware of their difficulty and that help is on its way.

1.2 Rescue

In each elevator there is a Help Button to enable a stranded person to call for help; however, if you discover that someone is stranded, advise Plant Operations at extension 33793 (emergency calls, 24 hours) immediately.

1.3 Power Failure

In the event of a power failure, the elevator will remain stationary and, after a brief interval, emergency power will provide lighting in the elevator.

1.4 Elevator Malfunction

Notify Library Facilities Services Section of Library Technology and Facilities Services at extension 32649 of any elevator malfunction (e.g., elevator not stopping at appropriate level, doors not closing properly). After hours, notify Plant Operations at extension 33793.

Section 2: Fire

2.1 Fire Alarms

When the fire alarm sounds:

  1. Leave the building as quickly and safely as possible, even if there is a suspicion of a false alarm.

  2. Do not attempt to use the elevators (the main elevators in the Dana Porter Library automatically go to and remain on the 2nd floor in the event of an alarm, and must not be used).

  3. Do not re-enter the building until authorized to do so by the Fire Department and/or UW Police.

In addition:

  1. If safety permits, a pre-designated staff member should lock cash boxes and confidential files in those departments that have them. Office doors should be left closed, but unlocked.

  2. At least one staff member in each department should be designated by the department head as a departmental Fire Warden. Fire Wardens are responsible for ensuring that all the occupants of designated areas are evacuated during emergencies and drills (see 2.7 Fire Wardens for details).

  3. When practical, staff members from one department (or area of a department) should stay together outdoors until counted by their Fire Warden (see section 17 for list of Fire Wardens).

2.2 Smoke

If you are caught in smoke while leaving:

  1. Get low to the floor (where the cleanest air is), and crawl.

  2. Take short breaths (through the nose) until you reach safety.

  3. Try to keep calm and think before you move.

2.3 Discovery of Fire

If you discover fire (all fires, regardless of size) or smoke, and the alarm has not yet sounded:

  1. Activate the nearest fire alarm pull station (locations are shown on the attached floor plans).

  2. Use the nearest fire extinguisher (indicated on the floor plans) only if you feel you can safely fight the fire (e.g., a small one in a waste basket).

  3. If you cannot safely extinguish the fire, close the doors and evacuate the area. Do not use the phone.

  4. Lock the front doors and wait at the loading dock to the building for authorities.

2.4 General Instructions

  1. Do not attempt to extinguish a fire if you cannot safely do so.

  2. Familiarize yourself with the locations of fire alarm stations and extinguishers in your general area.

  3. All fumes should be reported to the Library Facilities Services Section at extension 32649 (do not leave voice mail; if no answer notify Library Office at extension 32282). After normal business hours notify Plant Operations at extension 33793.

2.5 Fire Extenguishers

In addition to the water hose¹ located at most of the fire extinguisher stations throughout the Library, there is a portable, pressure-charged fire extinguisher located at each station. These extinguishers are filled with water, dry chemical, or carbon dioxide.

Instructions about the type of fire that can be extinguished with the three types of extinguishers are printed clearly on the extinguishers:

2.6 Fire Safety for Persons with Disabilities

The following guidelines should be observed, where possible, to help ensure the safe evacuation of persons with disabilities:

  1. Fire Wardens will help with the safe evacuation of persons with disabilities who are capable of walking either independently or with assistance.

    Floors 6 - 10 in Dana Porter are not monitored by Fire Wardens; therefore it is strongly recommended that library users who would require assistance during evacuation be accompanied by a buddy while using these floors.

  2. Fire Wardens will assist a wheelchair user or other person with a disability into the nearest stairwell, reassure them that assistance is on the way, and:

    1. ask for a volunteer to stay with the patron with a disability.

    2. notify the Building Evacuation Coordinator (at annunciator panel) or dispatching emergency staff immediately of location of the patron with a disability. The Fire Department or emergency staff will arrange rescue.

If UW Police arrives at the scene first, seek assistance from them for rescue.

2.7 Fire Wardens

The safe evacuation of all occupants of a building in the event of an emergency is of prime importance. Fire Wardens are staff members who have accepted the responsibility of Warden’s duties in addition to their regular duties. Part of their responsibility is to help ensure quick and orderly building evacuations. The following procedures apply:

Be prepared

  1. Review and study the floor plan for your area of responsibility, the number of floor occupants, and the number of exits, for the purpose of dividing the population into groups or squads and formulating the traffic pattern for each group to primary and secondary exits. Record areas where evacuation may be difficult because of special locks or other hindrances. After 4:30 pm internal M-F/weeks and weekends lock-up, staff will need micro keys to access the loading dock area. UW Police always have keys.

  2. Daily examine and determine that all fire doors and stairs are maintained in the closed position and that no doors are obstructed or inoperable or locked in violation of fire regulations.

  3. Have available an updated list of personnel with physical disabilities who cannot use stairs unaided.

In emergencies

  1. Take any action necessary to prevent panic.

  2. Ensure that all persons on the floor are notified of the emergency and all are evacuated to safe areas. Have staff volunteers assigned to conduct a prompt search of the washrooms to ensure that all persons are out.

  3. Ask evacuees to stay at least 100 feet (30 meters) from the building.

  4. After the evacuation, conduct a head count, when practical, to ensure that all regular known occupants have been evacuated.

  5. When evacuation duties have been completed, proceed to the vicinity of the communication station to maintain communications with the Evacuation Coordinator and to receive and give instructions. The communication stations (or annunciator panels) are as below.

  6. Note and report to fire officers if any hazardous apparatus or equipment has been left on.

  7. Assist UW Police, as required, to ensure that only authorized personnel enter the premises.

  8. Prepare a written report of difficulties, ambiguities, and suggestions for improvement and forward to Health Services (copy to Carl Nagel, Library Facilities Services Section).

  9. Wear the Warden’s identification cap, whenever possible.

NOTE: Should the Fire Warden encounter resistance to efforts to evacuate the building, he/she is not expected to take any action that might endanger the safety or life of any library staff or patron. The Warden’s responsibility ends with ensuring that people are aware that evacuation procedures are in effect. Should people refuse to leave, report the number of persons and their location to the Evacuation Coordinator, Security, Police, or Firefighters as soon as possible.

Section 3: Procedures for Evacuating Buildings

Fire Wardens or UW Police may announce the need to evacuate the building. When this occurs, the following procedures should be observed:

  1. Follow the instructions of the Fire Wardens or UW Police and leave the building as quickly and safely as possible using the designated exit.

  2. Do not attempt to use the elevators; use the stairwells only.

  3. Do not re-enter the building until authorized to do so.

In addition:

  1. If safety permits, a pre-designated staff member should lock cash boxes and confidential files in those departments that have them. Office doors should be left closed, but unlocked.

  2. Staff members who are designated as “Fire Wardens” are responsible for ensuring that their assigned areas are cleared.

Section 4: Emergency Lockdown Procedures

What is a lockdown?

A lockdown is implemented when there is serious security risk (e.g., violent or armed intruder) to building occupants.

How will I know a lockdown is occurring?

You will be notified of a lockdown procedure through one or more of the following:

Lockdown Steps

If a lockdown occurs, secure an area as follows:

  1. Move immediately to the nearest room you feel is safe with as many people as possible;
  2. Lock and barricade the door;
  3. Turn off the lights or maintain minimal lighting;
  4. Cover all windows with blinds, curtains, etc.;
  5. Keep back from windows and doors;
  6. Lie flat on the floor or take cover out of sight;
  7. Turn off cell phones except to report injured people;
  8. Keep calm and quiet; and;
  9. Stay in the room until police arrive. Remember it may be several hours before you can be safely evacuated.

What if someone is injured?

Follow these steps when safe to do so:

Who to call?

Call 911 or UW Police 519-888-4911 or ext. #22222 for all campuses. You may access www.uwaterloo.ca for information.

What to report? When contacting authorities, report the following:

  1. Your specific location, building name and office/ room number;
  2. The number of people at your specific location;
  3. If there are injuries, the number and types of injuries; and
  4. If you have seen an assailant or identified a threat:
    • location and number of suspects;
    • direction of travel;
    • their clothing and description;
    • their identity if known;
    • any weapons or accessories (e.g., backpack); and
    • any unusual or threatening sounds (e.g., gunfire or explosion)

Section 5: Flooding or Water Leaks

5.1 Flooding

  1. Do not enter flooded area; there may be danger of electrical shock.

  2. Report flooding immediately to Library Facilities Services Section at extension 32649 (do not leave voice mail; if no answer, notify Plant Operations at extension 33793). After normal business hours, notify Plant Operations and one of the people listed in 5.2 below.

5.2 Water Leaks

  1. Report water leaks immediately to Library Facilities Services Section at extension 32649 (do not leave voice mail; if no answer, notify Plant Operations at extension 33793). After normal business hours, notify Plant Operations and one of the people listed below.

  2. Plastic range covers are strategically placed on each floor of all Libraries. In addition, each staff workstation has been supplied with smaller plastic covers. If it is safe to do so, use these plastic covers to protect water threatened material and equipment.

Section 6: Fumes

6.1 Suspicion of Fumes

  1. Clear the general area where fumes are suspected.

  2. Notify Library Facilities Services Section at extension 32649 (do not leave voicemail; if no answer notify Plant Operations at extension 33793). After normal business hours, notify Plant Operations.

  3. Call the Plant Operations emergency telephone number and give:
    1. a description of the circumstances
    2. the location
    3. your name

6.2 Procedure

  1. Should a person be ill or overcome by fumes, remove the victim from the vicinity of the fumes. Warning: Be careful. If someone has been overcome by fumes, you could be too. Call for an ambulance immediately by dialing 911. Be precise in stating your location (e.g., University of Waterloo, Dana Porter Library). Be sure to give the building name.

  2. Send the nearest available person to the building’s designated “Emergency Entrance” (see 6.3) to await the arrival of the ambulance (or other emergency vehicle) and to direct medical personnel to the patient.

  3. Notify the Library Office at extension 32282 as soon as possible.

Section 7: Injuries/Medical Problems

7.1 Unconsciousness, Severe Medical Problems/Injuries, Apparent Death

  1. Call for an ambulance by dialing 911.

  2. Be precise in stating the location of the emergency. Provide:
    University of Waterloo
    Building Name
    Library Name
    Floor number

  3. Send the nearest available person to the building’s “Emergency Entrance” (see section 6.3) to await the arrival of the ambulance (or other emergency vehicle) and to direct medical personnel to the patient.

  4. Also notify the Circulation Desk staff of the exact location, in the event that emergency personnel arrive there first. During evenings and weekends staff areas leading to the emergency entrance are locked and UW police need to open.

  5. Look for “Medic Alert” tags bearing the standard physician’s symbol. These may be located around the wrist, neck, or in a wallet. Such tags indicate special medical problems.

  6. Notify the Library Office at extension 32282 as soon as possible.

7.2 Minor Injuries

The following procedures should be followed in those cases of injury which do not warrant following procedures described above:

  1. If professional first aid attention is required, call Health Services at extension 33541 (8:30 am - 5:00 pm). If the injured person can safely reach Health Services he/she should be encouraged and assisted to do so. If required, UW Police (extension 22222) will dispatch a cruiser to transport a person with minor injuries to Health Services, or after hours to a hospital emergency department. If the UW Police are attending to other emergencies, they will offer suggestions, e.g., calling a cab.

  2. If the injury is sufficiently minor, administer first aid. Staff members with first-aid training are listed in section 15.

7.3 Computer Ergonomics

Spending increasing amounts of time in front of a computer is contributing to a corresponding rise in health concerns directly related to their use. The most common health concerns are repetitive strain injuries, general muscle strain and discomfort, as well as eye and vision problems.  If you are experiencing any of these problems, please let your Supervisor know.

Preventing the onset of problems is important. Workstation design, as well as proper work practices can assist in prevention. Health concerns and suggestions for addressing them are outlined in the UW Health & Safety Office's online manual available at:

Section 8: Personal Harassment/Deviant Behaviour

Harassment, in this context, is understood as undue or unreasonable interference with one’s work or working environment. If you are subjected to personal harassment or if you witness some act of deviant behaviour, take the following steps:

  1. Report the incident immediately to the Circulation Desk staff on duty, who will call the UW Police.

  2. Remain at the Circulation Desk until Security officers arrive so that you can identify the person if he/she tries to leave the building.

  3. Report the incident to your Department Head and to the Library Office.

  4. If you are unable to leave an area, call the Circulation Desk and request that he/she come to you.

Section 9: Problems Relating to Alcohol/Drugs

Should you observe or encounter a person who appears to be under the influence of alcohol or drugs:

  1. DO NOT CONFRONT THE PERSON.

  2. Leave the area as quickly as possible.

  3. Report the incident immediately to the Circulation Desk staff on duty who will, in turn, advise the UW Police.

  4. Wait at the Circulation Desk until the authorities arrive so that you can provide further information.

Section 10: Physical Plant - Damage/Malfunction

Should you discover any damage to the Dana Porter Library building (broken windows, doors, etc.), or should you observe that some fixture (toilet, water fountain, etc.) is not working properly, report this to Library Facilities Services Section at extension 32649. In the Davis Centre Library, these reports should be made to the Davis Centre Library Office at extension 33126. In the University Archives (COM), these reports should be made to Library Facilities Services Section at extension 32649.

Advise Plant Operations at extension 33793 of any damage or malfunction that is discovered in the evening or during the weekend, if it is such that it must be attended to immediately.

Notify UW Police at extension 22222 of any damage that might require investigating.

Section 11: Power Failure

11.1 Safety

In the case of a power failure, emergency power will provide limited lighting.

Elevators should be checked for stranded persons.

11.2 Operations

  1. If the failure occurs during daylight hours there will be enough natural light to continue most library operations. The University Librarian or his/her delegate will decide when/whether to discontinue operations.

  2. If the failure occurs at night or on weekends the Circulation supervisor on duty should report it to Plant Operations at extension 33793. If it can be determined that the failure might be prolonged, the supervisor should consult one of the people listed below to learn whether the Library should be evacuated:

Section 12: Takeover of Buildings/Sit-ins/Hold-ups/Robberies

12.1 Do Not Defend the Library

It is not your responsibility to act as a police officer. Resistance may only serve to increase the destruction and bring about bodily harm to staff and patrons. Therefore offer no resistance.

12.2 Call the UW Police . . . Discreetly. . . and as Soon as Possible

This should be done whenever a large crowd gathers, whether or not aggressive or threatening acts occur. Give the UW Police (extension 22222) the following information:

  1. Your location.

  2. Details regarding:
    1. the size and location of the crowd and the damage being done
    2. whether the building is occupied
    3. whether the crowd seems rational and organized
    4. your name
    5. any description of building occupants/thieves/etc. that may be useful to the UW Police.

The UW Police will notify the Regional Police if necessary.

12.3 Notify the Library Office

Section 13: Threats, Bomb

If you receive a call regarding a bomb threat:

13.1 Get Maximum Information

Keep the caller on the telephone if possible without endangering yourself and occupants of the building and WRITE DOWN as much of the following information as you can obtain:

  1. The time the bomb is set to go off

  2. The location of the bomb (building, floor, room)

  3. The kind of bomb

  4. The reason the bomb was set

  5. Any other information which might prove useful in finding the bomb or identifying the caller.

13.2 Call UW Police Immediately and Pull Fire Alarm

  1. If the alleged time of the explosion is imminent (within 15 minutes of the call), advise UW Police at extension 22222 that the fire alarm will be set off immediately to evacuate the building and then activate the fire alarm.

  2. If more time is available, report the alleged location and time of explosion immediately.

  3. Give your name.

  4. Report the incident to your Department Head and to the Library Office.

  5. Wait outside the emergency entrance of the building for authorities in order to provide further information.

Section 14: Guidelines for Storm Situations

14.1 When Storms Commence During the Work Day

A decision to "close" during the day will be made by the Provost, in consultation with as many of the following as can be reached: the Director of Police & Parking Services, Director of Communications & Public Affairs, and Director of Custodial & Grounds Services. Such a decision will specify whether the closing is "immediate" or at a stated hour and will be conveyed to departmental offices as expeditiously as possible, as well as being posted to UW's homepage and conveyed to local radio stations. Unless otherwise specified, the closing continues from that hour until 7 a.m. the next day. At the designated hour, staff and faculty (except those providing "essential services") are entitled to leave, without loss of pay. As seems reasonable, a Department Head may give permission, for staff and faculty to leave sooner. A class or examination that would continue past the specified closing hour is cancelled in its entirety, as are scheduled evening classes.

14.2 When Storms Commence at Times Outside the Work Day¹

A decision to "close" will be conveyed by the Provost to the Director of Communications & Public Affairs, who will arrange to: post details on UW's homepage, amend UW's automated telephone greeting, and notify local radio stations as necessary. The Provost, with assistance from C&PA, or others as required, will notify Executive Council, the Faculty and Staff Association Presidents and the student Presidents (Federation of Students, Graduate Student Association) who, in turn, will notify others under their jurisdiction. In such circumstances, individuals are to consult UW's homepage, tune in to a local radio station or call UW's Infoline (1-866-470-0910) to learn whether UW is closed.

¹Note: Notice of a campus closing will be posted on UW's homepage, normally by 7:00 a.m., remaining in effect until 7:00 a.m. or later the next day. Radio stations that can be expected to carry announcements include CKGL (570 AM), CKKW (1090 AM), CHYM (96.7 FM) and KOOL-FM (105.3 FM).

Assistance - Services for Persons with Disabilities
